When using our online platform to
submit an application:
Visit Leave a Legacy to get started on your application. A link to the application is available.
Application forms are accessible from a computer, tablet, or smartphone.
We recommend using the most current version of Mozilla Firefox or Google Chrome, which works on both Macs and PCs.
To get started on the online application, visit this direct link: philadelphia250.slideroom.com. Click “Apply Now” to create your SlideRoom account to begin the process. Applications can be previewed by selecting an “available program,” then click “Preview Full Application.”
To save your changes, click the “Save Changes” at the bottom left of the page. This form will auto-save.
The online application uses plain text and may count characters differently than programs like Microsoft Word. We recommend typing your answers directly into the application. If you do copy and paste, please avoid formatting such as bold text. To create a bulleted list, use asterisks (*) or hyphens (-).
When you submit your application, you can print a copy of your application and you will receive a confirmation email.
Required fields: Required fields are indicated with a black asterisk (*). You must fill in all required fields in order to submit the application.
Characters counts: Some fields have a character count limit. Be mindful of the character count on the bottom right of the field.
